Six kinds of skilled care, delivered at home
Every visit follows a plan of care approved by your doctor or another allowed practitioner, such as a nurse practitioner. These are the six services our team provides.
Physical therapy
Rebuild strength, balance and walking ability at home, with exercises matched to your stairs, hallways and daily routine.
Occupational therapy
Practice everyday tasks like bathing and dressing again, and set up your home so those tasks are safer and easier.
Speech therapy
Speech-language pathology for speaking, understanding, memory and swallowing — often after a stroke or serious illness.
Medical social services
Help with the social and emotional side of an illness, including counseling and finding resources in your community.
Home health aide
Part-time help with bathing, grooming and moving around the house while you are also receiving skilled care.
How it works
How home health care starts
Medicare requires a doctor’s involvement before home health begins. Here is what happens between that visit and your first day of care.
A doctor’s visit and order
Your doctor, or an allowed practitioner such as a nurse practitioner, sees you in person and orders home health care.
The referral reaches us
Your doctor’s office or hospital faxes the referral to (818) 898-1145. You or a family member can also call (818) 898-1100.
Your first home visit
A Rinaldi nurse or therapist visits your home to assess your needs — typically within 24–48 hours of the referral.
Care follows your plan
Visits follow the plan of care your doctor approves. If you still need care, the plan is recertified at least every 60 days.

Who qualifies for Medicare home health care?
Original Medicare covers home health care when all four of these are true.
- You are homeboundLeaving home takes a considerable and taxing effort, and you need help, a device or special transportation to do it — or your doctor advises against leaving.
- You need part-time skilled careIntermittent skilled nursing, physical therapy or speech-language pathology, or a continuing need for occupational therapy.
- A doctor or allowed practitioner orders itThey see you in person — within 90 days before or 30 days after care starts — and approve your plan of care.
